If you install and run Run Plugin By Name as Roy suggested, it will tell you if you have any duplicate plugins, and duplicate plugins should be removed. If there is no user copy, you might still have a duplicate at another location.
If you see "User Copy" at the name then there is a replacement and you should remove the user copy (Delete in Edit Plugin will do that for you). One way to tell is to go to File.Plug-ins>Edit Plug-ins, then go down to Transformations and look at Double Note Values (see screen shot).
